Bankruptcy, Contract, Civil Suit, or Foreclosure information. This includes all contact person(s).
Ordinance part (You MUST contact the Planning and Zoning Department for this information.)
Proposed zoning change (if any)
Estimated increase in vehicle trips per day
Type of vehicles using facility
Reason for proposed special use
Any studies, drawings, etc (these may be e-mailed to firstname.lastname@example.org)
All of the above information is required to complete this permit application. If you do not have all of the above information, you will be unable to complete your permit.
